Access to Information Forms

Audience-specific forms are available to help you obtain the information you need, whether you are an access to information and privacy (ATIP) specialist or a member of the general public.

Links to forms for ATIP specialists:

  • Personal Information Bank Registration Form (TBS/SCT 350-84) – ATIP Offices complete this form to register a personal information bank (PIB). Once the PIB has been registered by the Treasury Board of Canada Secretariat, the institution is responsible for including the information in their chapter of the relevant Info Source publication.
  • Statistical Report on the Access to Information Act (TBS/SCT 350-62) – ATIP Offices complete this report annually. This report is part of the larger annual report of government institutions.
  • Statistical Report on the Privacy Act (TBS/SCT 350-63) – ATIP Offices complete this report annually. This report is part of the larger annual report of government institutions.
  • Training Registration Form – Fill out this form to participate in training offered by the Information and Privacy Policy Division of the Secretariat.

  • Access to Information Request Form (TBC/CTC 350-57) – This form is used for public requests for government information. Complete steps 1 through 4 to get the information you need.
  • Personal Information Request Form (TBC/CTC 350-58) – This form is used for public requests for personal information the government has about you, as established under the Privacy Act. Complete steps 1 through 4 to get the information you need.
  • Record Correction Request Form (TBS/SCT 350-11) – If you believe there are errors or omissions in the information the government has about you, use this form to request corrections. The completed form should be sent to the government institution that holds your personal information.
