Subject: Amended Relevé-1 and T-4

Employee on Paid Leave as a Result of an Injury on Duty

In response to your enquiry concerning an amended Relevé1 and T4 for an employee on paid leave as a result of an injury on duty, I have confirmed the following information with the Commission de la santé et de la sécurité du travail (CSST) of Québec.

Amounts applicable for this claim were credited to the employer during previous taxation years. In certain situations the taxation years and/or the amounts for the Relevé 1 and T4 are not the same for the employee. The difference is a result of Canada Revenue Agency's regulations requiring the employer to report deemed income in the year it was credited. Please request your pay office to produce amended Relevés 1 and T4's as follows:

Employee name:

PRI:

File Number:

Taxation Year Relevé 1 T4
#### $xx,xxx.xx $xxx.xx
### $xx,xxx.xx $xx,xxx.xx
#### $x,xxx.xx $x,xxx.xx

If you require additional information, do not hesitate to contact me at (your phone number).
